All India Institute of Medical Sciences, Madurai (AIIMS Madurai) was Announced in 2015 and the institute established in 202.  It is a Public hospital and Medical school.

The AIIMS was officially announced on 28 February 2015, in the budget speech for 2015–16, where the Minister of Finance Arun Jaitley announced five more AIIMS, in Jammu & Kashmir, Himachal Pradesh, Punjab, Assam and Tamil Nadu and an "AIIMS-like" institute in Bihar. Tamil Nadu had proposed five sites to set up an AIIMS. Besides Thoppur, Sengipatti (Thanjavur), Pudukottai town, Chengalpattu (Kanchipuram) and Perundurai (Erode) were considered. In 2018, the union cabinet gave an official approval for the location, on a 222-acre plot in the Thoppur, near Madurai.

 
By January 2019, Prime Minister Shri Narendra Modi had laid the foundation stone of AIIMS Madurai and the boundary wall was completed. The state government handed over the land to the Government of India in February 2020. In March 2021, the funding agency, Japan International Cooperation Agency (JICA), extended financial assistance. The loan agreement was signed in February 2022. The Japan International Cooperation Agency (JICA) informed Minister for Health and Family Welfare of Tamil Nadu Mr. Ma. Subramanian in February 2023 that the construction of AIIMS Madurai would begin by the end of 2024 and be completed by 2028.

 
Thoppur, the village near Madurai where Tamil Nadu's very own AIIMS will come up, is located on the National Highway connecting Madurai with Kanyakumari. The total built-up area, excluding site development area) is about 2,00,851 square metres, the tender document says. The construction of the complex, which is expected to begin by December this year, will take about 33 months for completion.  With work on the institute yet to start, there is little activity in the area.

 
Though the construction is yet to start, the medical college of AIIMS, Madurai is temporarily functioning from the Ramanathapuram Government Medical College. The permanent campus will be functional in 2025 and by 2028 AIIMS Madurai will be fully functional.  The Union Government has appointed Dr M Hanumantha Rao and Dr Prashant Lavania as the Executive Director and President of AIIMS, Madurai.

 
The AIIMS Madurai will have a hospital building spread over an area of 108,325 square metres with 870 beds including out-patient department, and Emergency, an Ayush block, a teaching block, an auditorium with a seating capacity of 750, conference halls, separate hostels for girls and boys, service block, Director’s bungalow, and residential complexes for professors and other staff.  The AIIMS Madurai will have 15-20 super specialty departments.  According to the current operational AIIMS data, it is expected that the hospital will cater to around 1500 OPD patients per day and around 1000 IPD patients per month. 

 
Like all other AIIMS, the yearly MBBS intake is 50 by the AIIMS Madurai-run medical college and the institute selects students for MBBS course through NEET(UG).   All India Institute of Medical Sciences, Bhubaneswar is one of the apex healthcare Institutes established by the Ministry of Health & Family Welfare, Government of India under the Pradhan Mantri Swasthya Suraksha Yojna (PMSSY).

AIIMS Madurai, like other AIIMS institutions, conducts MBBS admissions based on NEET scores followed by centralized counseling conducted by the Medical Counseling Committee (MCC). Candidates must first appear for and qualify in the NEET-UG exam conducted by the National Testing Agency (NTA)

The institution is rapidly transitioning to its permanent Thoppur campus, spanning over 222 acres, with Phase I including an academic block, outpatient services, hostels, mess halls, essential service buildings, and more—expected to be operational by early to mid-2026 . The future campus plans include a 900-bed tertiary hospital featuring a dedicated 150-bed infectious diseases block, 750-seat auditorium, separate hostel facilities for students and staff, modern lecture halls, labs, library, gym, auditorium, and sports infrastructure—all built to global sustainability standards aimed at achieving IGBC Gold certification

The nearest major bus terminal is the Mattuthavani Integrated Bus Terminus, approximately 120 km away, providing extensive intercity connectivity.
